They're two completely different things. Montepulciano (the place) is a Tuscan hilltop town whose flagship wine, Vino Nobile di Montepulciano DOCG, is made from the Sangiovese grape — locally called Prugnolo Gentile. Montepulciano d'Abruzzo is a red wine from the Abruzzo region on Italy's east coast, made from the unrelated Montepulciano grape. Same name, different grape, different region. Both are Tuscan hilltop towns about an hour apart, both make a flagship Sangiovese-based DOCG red, but the wines are different. Montalcino's Brunello is made from 100% Sangiovese Grosso, aged a minimum of 5 years, and tends to be more powerful and structured. Florence to Montepulciano is about 115 km / 1h45–2h by car via the A1 south to the Valdichiana exit. By public transport: take the Trenitalia regional train from Florence to Chiusi-Chianciano Terme station (about 1h30), then the Tiemme bus to Montepulciano (about 50 minutes). Rome to Montepulciano is about 180 km / 2h–2h30 by car via the A1 north to the Chiusi-Chianciano exit. By train, take Trenitalia from Roma Termini to Chiusi-Chianciano Terme (around 1h15 on the fast service), then the Tiemme bus to Montepulciano (about 50 minutes). Self-drive gives you the most flexibility for visiting countryside wineries; if you don't want to drive, book a small-group day trip.
